Request a Letter of Medical Necessity (LMN)
Like a doctor's note, Sika requires an LMN or a Letter of Medical Necessity. In the Sika Health portal, you'll take a quick survey of symptoms will see if you prequalify for a Letter of Medical Necessity from Sika. LMNs are issued by a healthcare provider to outline why a particular product or service is medically necessary for your health and qualifies the purchase to be HSA/FSA eligible. LMNs demonstrate that an expense is needed to diagnose, treat, or alleviate a medical symptom rather than for general health or wellness purposes. Eligible products must treat, cure, prevent, diagnose, or alleviate a medical need.
Verify Your Identity and Chat with a Healthcare Professional
Complete a phone verification and upload your ID to verify your identity. After this, you'll submit your forms in the Sika Health portal and one of their healthcare providers will reach out over text to determine your LMN eligibility.
Completing the Purchase, If Approved
Once you select Continue with Sika, you'll be taken to a third-party service for HSA/FSA payment, where a review order window will display:
-A subtotal for items you can purchase with your HSA or FSA card.
-A subtotal for items that aren't eligible for HSA or FSA payment.
You must use a different credit or debit card to purchase these items. Once you add an HSA or FSA card and a different credit or debit card, select Complete Purchase.*
Order Confirmation
Sika will send a confirmation email after you complete your purchase. A confirmation email from support@sikahealth.com will be sent to the email address provided in the original order.
Important Considerations To Note
Your administrator will process and evaluate your claim. Please note that some HSA/FSA benefits administrators may incorrectly deny reimbursement, so before purchasing a product with an LMN you should consider contacting your administrator to determine if your purchases will be covered/reimbursed. What is a Letter of Medical Necessity?
An LMN, or Letter of Medical Necessity, is like a doctor’s note. Why do I need a Letter of Medical Necessity to purchase Human Touch's products?
If you want to use your HSA/FSA funds to purchase [Merchant’s] products, you’ll need a Letter of Medical Necessity (LMN), and we’ve partnered with Sika to help you with this.
Certain products are eligible for purchase with HSA/FSA funds if they treat, cure, prevent, diagnose, or alleviate a medical need. For instance, items like bandaids are always eligible as they serve a solely medical purpose. However, there are “dual-eligible” products that can be used for general health and wellness or for specific medical purposes—if recommended by a healthcare provider based on your individual needs. Take iron supplements, for example; many use them to support their overall health. Yet, for someone with anemia, iron supplements become crucial. In such cases, individuals can use their HSA/FSA funds if their healthcare provider issues an LMN, recommending the supplements specifically to address their anemia. Human Touch products are considered "dual-eligible," and require an LMN for purchase with HSA/FSA funds.
How do I get a Letter of Medical Necessity?
Choose Sika as your payment method to use your HSA/FSA at checkout for your qualified purchase. You’ll fill out a quick survey to tell Sika's provider team about your qualifying symptom(s). Once you’re approved, Sika will email your receipt and next steps for substantiation.
How long does it take to get approved for a Letter of Medical Necessity?
The Sika healthcare provider team strives to provide a response within 12 business hours after you submit your LMN intake forms.
What information will I need to provide for a Letter of Medical Necessity?
To see if you’re a candidate for a Letter of Medical Necessity, similar to other telehealth experiences, you’ll be asked to share your name, contact information, date of birth, and information about the symptoms you are experiencing and your medical history. Sika will also verify your phone number and ask you to upload a photo ID to verify your identity.
The information you provide in the LMN process will be kept confidential and shared only with Sika healthcare providers to evaluate your eligibility for an LMN. For more information, see the Sika Health privacy policy.
How long is a Letter of Medical Necessity valid for?
Letters of Medical Necessity are valid for 12 months from the day they are signed and written.
Please note, LMNs cannot be used retroactively for purchases made prior to the date they are signed.
How do I submit my Letter of Medical Necessity to my HSA/FSA provider?
After you complete a purchase using an LMN, you will need to submit your Letter of Medical Necessity and your Human Touch receipt to substantiate the purchase if you paid with your HSA/FSA or to get reimbursed if you paid with your credit/debit card. To do this:
- Log into your HSA/FSA account on your benefit administrator’s website
- Find the Reimbursement or Claims section
- Upload your receipt and Letter of Medical Necessity

Why wasn’t I approved for a Letter of Medical Necessity?
Sika’s independent healthcare provider may determine that you do not have symptoms demonstrating a medical necessity for a product or service. While you cannot use your HSA/FSA funds now, you can still use another payment method to complete your purchase on our site. We do not recommend reapplying.
